Prepare for the SMA

Once you've set the date for your SMA. preparation should begin at least 4 weeks out. Preparation requires input from a number of key contributors.

Admin

    Organise catering/refreshments

  • 4 wks

    Enter the screening term for each patient to monitor patient preparation checklist

  • 2 wks

    Prepare the patient information package

    • confidentiality forms
    • SMA information
    • Patient education materials
    • Patient self-management record
  • 2 wks

    Send patients SMA information

Nurse

  • 4 wks

    Organise patient tests/diagnostics

  • 2 wks

    Prepare and print any patient handouts/clinical information for patient package or as group handouts

  • 2 wks

    Update care plans and patient notes

  • 1 wk

    Prepare patient files

  • 1 wk

    Record tests and diagnostics in patient notes

    Run screening term query and follow-up patients who have not completed activities.

Patients

  • 2 wks

    Complete pre-SMA tests and diagnostics

  • 2 wks

    Make time available for the SMA

  • 1 wk

    Think about questions for their consultation

  • 1 wk

    Organise a support person if necessary

  • 1 wk

    Read patient information package and sign confidentiality form

Clinician

  • 4 wks

    Select patient group (25-30)

    Phone patients for first SMA

  • 2 wk

    Order pre-SMA tests and diagnostics

  • 1 wk

    Decide which patients may need a physical examination prior, during or post SMA.

Location and time considerations:

For some communities and rural practices, it may be more appropriate for the SMA to occur where a patient group is working or in another community setting rather than expecting them to attend the practice. SMAs can be mobile. They could be delivered on site to forestry workers, factories, maraes or large organisations. Alternatively, running the SMA in the evening or weekend is another alternative to ensure attendance and ultimately behavioural change.
